​The current literature on acid-related diseases is dominated by Delphi consensuses. These consensuses represent a compilation of older views on the pathogenesis of gastroesophageal reflux, which is considered complex and multifactorial. This article, for the first time, proposes an evidence-based hypothesis for the etiology of acid-related diseases, including GERD. This hypothesis is offered for discussion, and if it contradicts even one piece of scientifically sound evidence, it will be revised or rejected.

Obstruction of the third part of the duodenum is thought to be the result of intestinal compression between the aorta and the superior mesenteric artery. A review of the literature and radiographic studies demonstrates that duodenal stenosis is caused by dyskinesia of the sphincter of Ochsner.
